Changes in health insurance premiums. There is a decision of the members of parliament

The legislators endorsed a bill aimed at decreasing the health insurance contribution. The legislation proposes a dual-element assessment base for contributions. The bill is now set to be reviewed by the Senate.
The Sejm had earlier dismissed the committee’s amendments. A proposal to completely reject the bill also failed during the voting process.
Who supports it, who opposes it?
A total of 428 deputies voted in favor of the bill: 213 supported it, 190 opposed it, 25 abstained, and 32 did not cast a vote.
The new regulations received backing from 152 members of the KO club, including Health Minister Izabela Leszczyna. In March, Leszczyna had stated that she would definitely oppose cutting health insurance contributions for entrepreneurs, claiming that the National Health Fund „would certainly struggle with that.” One MP from the Coalition, Franciszek Sterczewski, abstained, while 4 MPs chose not to vote. Additionally, 29 MPs from Polska 2050-TD supported the act (3 MPs did not vote), as did 30 MPs from PSL-TD (2 MPs did not vote). One independent MP, Adam Gomoła, also voted in favor, while 2 other independents abstained.
Opposition came from 166 PiS MPs, with 1 MP, Andrzej Gawron, voting in favor, 7 abstaining, and 16 not voting. The new regulations were also opposed by 20 MPs from the Left (1 person did not vote) and 4 MPs from the Razem group (with 1 member from this group also not voting).
12 MPs from the Confederation abstained from the vote, and 4 did not participate.

Modifications to health insurance premiums
The proposal includes the establishment of a two-part structure for determining the health insurance contribution for entrepreneurs—up to a certain threshold, it would be a flat rate, and for any income exceeding that threshold, a percentage would apply.
The flat-rate portion of the contribution will be set at 9% of 75% of the minimum wage. The second portion will be based on income exceeding 1.5 times the average monthly wage for entrepreneurs who are taxed on a tax scale or flat tax (with a 4.9% rate).
For entrepreneurs subject to a flat tax, the second portion will be calculated on income exceeding 3 times the average monthly salary (at a 3.5% rate). The bill also proposes reducing the contribution to 9% from 75% of the minimum wage (currently 9% of the minimum wage) for entrepreneurs taxed under a tax card. The government proposal eliminates the option of deducting paid health insurance contributions from income tax.

Impact on the state budget
The adjustments to the health insurance contribution are projected to cost the state budget PLN 4.6 billion, with the regulations set to take effect at the beginning of 2026.
The Ministry of Finance estimates that around 2.45 million entrepreneurs will gain from this reform; taxpayers with the highest incomes who report their taxes on a flat rate basis will pay higher contributions than they do now.
Andrzej Domański’s remarks on the act
– Our government is progressively reversing the adverse effects of the Polish Deal. Regarding the health insurance contribution, we first stopped calculating it based on the sale of fixed assets, then we lowered the minimum contribution. Today, we are implementing a solution that will benefit 2.5 million Polish entrepreneurs – stated Finance Minister Andrzej Domański before the vote in the Sejm.
– I want to make it clear that all expenses related to this project will be covered by the state budget, and the National Health Fund will incur no costs from this solution – he continued.
He also addressed „Razem” MP Marcelina Zawisza, who proposed a motion to reject the project entirely. – The surpluses for the previous year have been fully settled – Domański remarked.
Contribution to the National Health Fund and access to costly therapies
Individuals who contribute to health insurance are covered under the public healthcare system. The insured have rights to treatments within the so-called basket of guaranteed services, which include medical consultations, procedures, surgeries, paramedic interventions, and medications, with reimbursement decisions made by the Ministry of Health.
